Complex behavioral health support at GuaranteeCare is for adults who need greater supervision, consistency, and individualized planning than a typical residential setting provides — often after hospital discharge, a placement change, or when current supports are no longer enough. The house stays small on purpose: six residents, familiar staff, and a known daily rhythm so behavior and safety plans can be followed carefully while the person still lives with dignity.

In Washington, many adults with significant behavioral health needs receive residential support through Community Behavioral Health Supports (CBHS) and Expanded Community Services (ECS). We welcome families, guardians, DSHS case managers, and hospital discharge planners when those pathways — and our Marysville home — are the right fit.

These are Washington long-term care supports for adults with complex needs in licensed residential settings. The state determines eligibility; when a person is approved and GuaranteeCare is the right home, we provide the day-to-day support that makes the plan work.

CBHS

Community Behavioral Health Supports

A Medicaid state-plan pathway focused on one-to-one supportive supervision for adults with complex behaviors related to a qualifying mental health diagnosis. At GuaranteeCare, that means attentive, consistent presence — helping the person stay safe, engaged, and supported through the day inside a small home.

ECS

Expanded Community Services

A higher residential support level inside an Adult Family Home. ECS typically includes personal care, medication oversight, 24-hour on-site supervision and response, and behavior support planning with the care team — while the person still lives in a house, not an institution.
